The statement sent to you when the insurance company settles an insurance claim, is your Explanation of Benefits (EOB). It takes time and patients to learn how to understand this document. Like so many thing in the insurance industry, there is no standard for this document. They all contain the same information, it is just organized and describe differently by different insurance companies. The best way I can think of to help you understand EOBs is to go through several example one at a time.

Each of the EOBs we will be looking at are actual patient EOBs that have had all the information about the patient remove. You will notice on each of our examples an area that contains information about the policy holder and the patient treated. Always check this information carefully. The insurance companies sometimes get this information wrong and you will need to contact them to get it corrected. Insurance companies will not change this information based on the word of your dental office, only the policyholder can correct these errors.
